Recently, I gave a presentation called \u2018Why Wellness Interventions Don\u2019t Work\u2019, to explore how the culture of medicine creates barriers to wellness.\u00a0 As a follow up, we gathered a group of 30+ physicians in a virtual workshop to reflect on the culture of medicine and their internalized expectations.<\/p>\n\n\n\n

First, we began by recognizing how the ideals of medicine e.g., \u2018Doctors don\u2019t make mistakes\u2019, \u2018Doctors don\u2019t complain\u2019, \u2018Doctors work until the work is done\u2019 become internalized through medical training. These ideals become one\u2019s identity e.g. \u2018I don\u2019t make mistakes\u2019, \u2018I don\u2019t complain\u2019, \u2018I work until the work is done\u2019. As physicians discussed their work environments, they gave many examples of failing to live up to such standards, resulting in feelings of shame and inadequacy. While the values in medicine of working hard and aiming for perfection are not \u2018bad\u2019, they are idealistic. Like the Northern star, principles are meant to guide us. However, when the ideal becomes internalized e.g. \u2018I am the Northern Star\u2019, we are set up to fail.\u00a0 When we fail to reach the impossible, we say \u2018It was my fault\u2019, and this is how the culture becomes embedded within us.<\/p>\n\n\n\n

Next in the workshop, we considered the cost of pursuing unrealistic standards. This is where people started to experience their emotions. Some felt sad about the lack of energy left for their family, some felt angry about the sacrifices they have made, and some admitted for the first time, the weight of their exhaustion. There were sighs of relief as stories came up for collective processing. There was a dawning of awareness that perhaps it didn\u2019t need to be like this, that perhaps the cost of giving everything is too much, perhaps new ways need to be found.\u00a0 The more each person shared, the more others stepped up to support them. Contrary to our fears, emotions do not mean weakness. Emotions are \u2018energy in motion\u2019, they create the momentum for change. Emotions don\u2019t lie, but impossible ideals stop us from listening to them.<\/p>\n\n\n\n

Next, I asked the group \u2018when you see yourself as having done your very best today, what do you notice?; \u2018can you see all the goodness you have offered\u2019? Sadness and exhaustion started to shift into lighter feelings. One person said \u2018pride\u2019 the next said \u2018I did a good job today\u2019, another said \u2018it feels easier to breathe when I think of the good I did\u2019, one more said, \u2018we need more positive affirmations of what we DO do\u2019. As people realized they are human and imperfect, they recognized the same is true of the systems they operate within. Like a light switch flipped on, self-generated solutions started to arise! Ideas on how to set boundaries, prioritize self-care, and the holding of complexity started to emerge. \u00a0Even in this virtual space, genuine warmth and connection could be felt and tears were shed as people let in some of this warmth.<\/p>\n\n\n\n

As the session closed, there was gratitude and thanks for the realness and connection offered.\u00a0 Somehow, through the process of acknowledging reality, participants began to see that change is possible and can start from within. They realized that emotions aren\u2019t the enemy and connection with others is what has been missing. There were final calls to find a way to ritualize this kind of sharing and a longing to honor those who have served in medicine when it comes their time to retire.<\/p>\n\n\n\n

Le degr\u00e9 de vuln\u00e9rabilit\u00e9 d\u00e9montr\u00e9 \u00e9tait inattendu et pourtant profond\u00e9ment r\u00e9parateur. J'ai le sentiment que, lorsque nous cr\u00e9ons un environnement psychologiquement s\u00fbr, des miracles peuvent se produire, des r\u00e9cits peuvent \u00eatre racont\u00e9s et les \u00e9motions peuvent circuler. Lorsque nous faisons briller une lumi\u00e8re dans l'obscurit\u00e9, nous reconnaissons notre douleur ET nos forces collectives. Le pouvoir de voir ce qu'il y a de bon en nous et chez les autres redonne \u00e0 l'humanit\u00e9 sa place dans la culture de la m\u00e9decine.<\/p>\n\n\n\n
